You can complete the player registration form and bring it with you. However, we have a new automated registration process. All players who have played for a city rec team before should already be in our database. Just show up and we’ll print your completed form. All you have to do is sign it and pay.

 

A player cannot be registered without the following three items:

· a completed Player Registration Form

· a copy of a valid birth certificate for the player

· proof of residency (if you’re new to the park or have recently moved)

· full payment of registration fee (checks are not cashed until registration closes)

 

Our birth certificate file was destroyed in the fall of 2007 so we need new copies from everyone.

All volunteers are required to submit a volunteer application form. For Little League baseball, all volunteers must also complete an additional form.
